Riverside Community Health Center is dedicated to building a patient centered health care home for you and your family. Our medical services focus on the delivery of wellness and preventive care, episodic sick care, chronic disease management, chiropractic care, laboratory services and women’s health.
Wellness and Preventive Care
Medical providers (doctors, physician’s assistants, and nurse practitioners) work with every patient to keep them healthy, reduce risk factors, and encourage healthy behaviors. Services include:
- Adult and childhood immunizations
- Physicals and regular screenings
- Women’s health screenings
- Well child check-ups
- Fluoride varnish applications for healthy teeth
Episodic Sick Care
When one of our patients becomes ill, our goal is to help them get better fast. Patient concerns that we treat on a regular basis include:
- Sore throats
- Muscle sprains and strains
- Other common illnesses
Chronic Disease Management
While a diagnosis of a disease does present challenges, in many cases these health conditions can be managed effectively. We work with our patients to help them understand their condition and take the necessary steps to limit any restrictions on their daily lives. Chronic diseases we help manage include:
- Hypertension (High Blood Pressure)
- Hepatitis C
- High Cholesterol
Riverside Community Health Center offers full laboratory services on site, in partnership with local laboratories. Our patients can have their labs conveniently drawn at the time of their medical visit. In addition, patients are welcome to have labs drawn on a walk-in basis, with no appointment necessary.
Lab services are contracted, meaning that each patient is billed directly for these services. A sliding fee scale for lab services is available.
Women’s Health Services
Riverside Community Health Center provides women’s health screenings, which include pap smears, pelvic and breast exams, and Sexually Transmitted Disease (STD) screenings. We offer a variety of contraceptive methods including birth control pills, Depo-Provera shots, IUDs, and implants to meet your family planning needs.

How do I become a medical patient?
Health Partners is always accepting new adult and pediatric patients. Please call to schedule an appointment with one of our medical providers to establish care with us. If you would like to transfer your health care from another medical provider, let us know and we will have you sign a release of information to get copies of your health records.
If I am sick today, can I get an appointment?
Yes, all of our locations are able to see a limited number of same-day appointments. Please call and let us know that you are sick and want to be seen today. However, if your regular medical provider is unavailable, you may see one of our other providers. As always, our Health Partners Quick-Care location at 1550 N. Main St. in Lima accepts walk-ins all day 8:00 a.m. to 8:00 p.m. (Monday – Saturday).
Can I come to Health Partners to be seen if my regular family doctor is unavailable?
No, Health Partners does not fill in for other medical providers in the community. You will need to contact your doctor’s office to find out where to get care.
